Lumi Health & Elevit Promotion Terms and Conditions
1. Promotion Overview
By participating in this promotion, you agree to the following terms:
● Order any Lumi Health Carrier Screening Test between 12:00am AEST on 15 July 2025 and 11:59pm AEST on 09 October 2025 via the Lumi Health website (https://www.lumihealth.com.au/) and have your free Lumi GP telehealth call booked (upon purchase).
● Also, receive a $10 off discount code for Elevit Pre-conception & Pregnancy vitamins 150 pack and 100 pack, redeemable at any participating Chemist Warehouse store within Australia or online at https://www.chemistwarehouse.com.au/.
● Discount codes must be used by 11:59pm AEST on 30 October 2025.
● Only the first 1,000 valid claims received will be awarded with the $10 discount code.
2. Eligibility
● Open to Australian residents aged 18 and over. Individuals who wish to claim the Medicare rebate for the testing must have a valid Medicare card and be female.
3. How to Redeem
Eligible customers will receive their discount code via email within 2 business days of their Lumi GP telehealth call.
● As per standard Lumi guidelines, each customer can only order one carrier screening test and will receive one discount code per eligible order.
● To redeem the discount at Chemist Warehouse:
o Participating Chemist Warehouse Stores: Present the valid discount code to the staff at checkout.
o Chemist Warehouse Online: Enter the discount code in the designated field prior to finalising the transaction.
4. Promotion Period
● Lumi Health Carrier Screening Test Order Window: Opens at 12:00am AEST on 15 July 2025 and 11:59pm AEST on 09 October 2025.
● Discount Code Expiry: Discount codes must be used by 11:59pm AEST on 30 October 2025.
5. Discount Code Details
● Discount Value: $10 off
● The discount code is not transferable, exchangeable, or redeemable for cash or any other products or services.
● The use of the discount code is subject to the availability of Elevit Pre-conception & Pregnancy vitamins 150 pack and 100 pack at Chemist Warehouse (in-store or online) and Chemist Warehouse’s standard terms of sale.
● Lost or stolen discount codes will not be replaced. It is the customer's responsibility to keep the discount code secure.
6. General Conditions
● The participants agree and acknowledge that they have read these terms and conditions and that entry into the promotion is deemed to be acceptance of these terms and conditions.
● The Promoter reserves the right to verify the eligibility of participants (e.g., Medicare/residential status and completion of the telehealth call). In the event that a participant cannot provide suitable proof as required by the Promoter to validate their claim, the participant will forfeit the offer in whole and no substitute will be offered.
● If there is a dispute as to the identity of a participant, the Promoter reserves the right, in its sole discretion, to determine the identity of the participant.
● The Promoter reserves the right to disqualify any claims or individuals who do not comply with these terms and conditions or who engage in fraudulent or manipulative activity. In the event that there is a dispute concerning the conduct of the promotion or claiming an offer, the Promoter will resolve the dispute in direct consultation with the participant. If the dispute cannot be resolved, the Promoter’s decision will be final.
● The Promoter reserves the right to modify, suspend, or cancel the promotion or these terms and conditions at any time due to unforeseen circumstances or as deemed necessary. Reasonable notice will be provided where possible.
● The Promoter accepts no responsibility for any tax implications that may arise from participating in the promotion. Participants should seek their own independent financial advice.
● Any guarantee or warranty given is in addition to any relevant statutory guarantees and warranties and nothing in these Conditions of Claim restricts, excludes or modifies or purports to restrict, exclude or modify any statutory consumer rights under any applicable law including the Competition and Consumer Act 2010 (Cth).
● If for any reason any aspect of this Promotion is not capable of running as planned, including by reason of computer virus, communications network failure, bugs, tampering, unauthorised intervention, fraud, technical failure or any cause beyond the control of the Promoter, the Promoter may in its sole discretion cancel, terminate, modify or suspend the Promotion and invalidate any affected claims, or suspend or modify the provision of discount codes, subject to State or Territory regulation.
7. Liability & Privacy
Lumi Health and Bayer Australia are not responsible for:
● Lost or stolen discount codes.
● The availability of Elevit Pre-conception & Pregnancy vitamins 150 pack and 100 pack at Chemist Warehouse stores.
● Technical issues that may affect a participant's ability to order a Lumi Health Carrier Screening Test, complete the telehealth call, or receive the discount code.
● Any issues arising from the use or misuse of the discount code at Chemist Warehouse, which are subject to Chemist Warehouse's terms and conditions.
Personal information collected by Lumi Health as part of this promotion will be managed in accordance with the Lumi Health Privacy Policy, available at Lumi Health Privacy Policy. By participating in this promotion, you consent to the collection and use of your personal information in accordance with that policy for the purposes of administering the promotion. The claimant agrees to the Promoter sending them electronic communications regarding this Promotion (for administrative purposes only) and acknowledges that such communications may not have an unsubscribe facility. If the claimant does not provide their personal information as requested, they may be ineligible to submit a claim a prize in the Promotion.
The Promoter and its associated agencies and companies will not be liable for any loss (including, without limitation, indirect, special or consequential loss or loss of profits), expense, damage, personal injury or death which is suffered or sustained (whether or not arising from any person's negligence or wilful misconduct) in connection with this promotion or accepting or using any offer (or recommendation). For the sake of clarity, this clause shall not apply where the Promoter has contributed to or caused such loss, expense, damage, personal injury or death and shall not apply to any liability which cannot be excluded by law (in each case the Promoter’s liability is limited to the minimum allowable by law).
8. Governing Law
● These terms and conditions are governed by the laws of Victoria, Australia.
● Any disputes arising out of or in connection with this promotion are subject to the exclusive jurisdiction of the courts of Victoria, Australia.
